Job summary

ALLIED SEARCH PTE. LTD. in Singapore invites applications for the Care Manager (Primary Care) role.

You will identify patients for care management, conduct evaluations, monitor indicators like blood glucose and HbA1c, and adjust care plans accordingly. You will also contribute to care program development, maintain confidentiality, guide junior staff, and participate in research to improve outcomes within chronic disease programs.

Qualifications

  • Possess a Diploma or Degree in Nursing with a relevant Advanced Diploma in Nursing Specialty.
  • Valid full registration with Singapore Nursing Board.

Responsibilities

  • Identify patients requiring care management through data review, referrals, and assessments.
  • Conduct comprehensive evaluations to determine patient needs and health status.
  • Monitor patients regularly using key indicators such as blood glucose levels and HbA1c.
  • Implement and adjust care plans in response to patients' progress and changing conditions.
  • Develop and track outcome indicators to ensure effective management of both clinical and non-clinical results.
  • Contribute to the development and continuous improvement of care programs and protocols.
  • Maintain patient confidentiality and ensure accurate, complete documentation.
  • Assist in creating and updating disease management guidelines, standards, and procedures.
  • Conduct audits of care management documentation to identify gaps and opportunities for improvement.
  • Participate in research activities and integrate evidence-based findings into practice.
  • Perform data analysis to support clinical pathway development and report on program outcomes.
  • Supervise and mentor junior care managers and nursing staff.
  • Conduct performance appraisals and provide constructive feedback to support professional development.
  • Oversee support group activities and facilitate patient recruitment for chronic disease programs.
  • Address workflow issues related to chronic patient management.
  • Monitor productivity and performance, providing coaching and guidance as needed.
  • Any other ad hoc duties as assigned.
