Job summary

UNITED MEDICARE PTE LTD is seeking an ACP facilitator to coordinate between residents, next-of-kin, and clinical staff for meaningful Advanced Care Planning discussions in our Singapore facilities.

You will support residents in expressing their values and care preferences, guide NOK through ACP processes, and work with doctors, nurses, CARES and other healthcare professionals to keep ACP records up to date. Multilingual abilities are a plus.

Qualifications

• Degree in Social Work, Counselling, Psychology or Nursing.
• At least 2 years' working experience in healthcare setting.
• Prior training in National ACP Facilitation.
• Knowledge in National ACP IT system (Compulsory).
• Proficient in Microsoft Office Suite (Excel, Word, PPT etc.).
• Ability to work independently with initiative, and effectively as part of a team.
• Excellent organisational and interpersonal skills.
• Ability to think quickly and react on the spot.

Responsibilities

• Coordinate between residents/their Next-Of-Kin and certified ACP facilitator for meaningful Advanced Care Planning discussion.
• Support residents in expressing their values, wishes and preferences for future care.
• Provide support and guidance to NOK regarding ACP processes and care preferences.
• Coordinate with doctors, nurses, CARES and other healthcare professionals for ACP-related matters.
• Ensure ACP discussions, decisions and relevant documents are accurately recorded and updated.
• Follow up on residents' ACP plans and facilitate review when there are changes in health or care needs.
• Identify and escalation complex medical, psychosocial or family issues to the appropriate healthcare professionals.
• Conduct or support ACP awareness sessions for residents, families and caregivers.
• Promote ACP as an ongoing process that can be reviewed when an individual's circumstances, values or preferences change.
