Job summary

THE SUPREME HR ADVISORY PTE. LTD. is seeking a qualified Social Worker to support intake screening, assessments, and care planning for elderly clients in a Singapore facility.

You will collaborate with nursing staff to identify psychosocial needs and ensure smooth admission and discharge processes. Responsibilities include conducting ACP discussions, coordinating with community resources, and maintaining comprehensive documentation.

Qualifications

• Bachelor Degree in Social Work or related field.
• Registered Social Worker with the Social Work Accreditation and Advisory Board (SWAAB).
• 3–5 years of relevant experience, preferably in a hospital or community hospital setting.
• Experience working with elderly and people with dementia.
• Certification as an Advance Care Plan Facilitator is an advantage.

Responsibilities

• Intake, Assessment and Admission Support: partner with Nursing staff to conduct intake screening for prospective residents/clients.
• Perform psychosocial assessments covering residents' and families' social, emotional, financial, caregiving and support needs.
• Identify residents and families requiring ongoing psychosocial support.
• Participate in pre-admission and admission assessments.
• Support newly admitted residents and families during admission.
• Conduct InterRAI assessments and related evaluations.
• Document assessments, interventions and care plans in the designated systems.

Education

Bachelor Degree in Social Work / BA (Major in Social Work) / Graduate Diploma in Social Work
