Job summary

JurongHealth Campus is seeking a healthcare professional to facilitate Advance Care Planning (ACP) for targeted patient groups and community partners. You will advocate ACP to patients, caregivers, healthcare professionals and the public, and support integration of ACP across JHC and NUHS in line with national guidelines.

You will contribute to ACP discussions, ensure documentation is accessible to care teams, and collaborate to improve ACP implementation and data reporting across platforms.

Qualifications

• Bachelor’s degree in Nursing, Psychology, Social Work, Counselling or related Allied Health field.
• Candidate should be a keen learner, critical thinker and team player.
• Good interpersonal, communication and multi-tasking skills.
• Proficiency in local languages/dialects and MS Office applications.
• Experience in Psychology/Counselling or related fields; familiarity with local healthcare/community services is a plus.

Responsibilities

• Contribute to patient care by facilitating ACP discussions and completing advance care plans that meet patients’ needs.
• Ensure patients and families have access to ACP documentation by providing soft/hard copies.
• Provide timely access to ACP documentation for care providers via the ACP Discussion Worksheet.
• Facilitate regular reviews and updates of ACPs with patients or nominated healthcare spokespeople.
• Support data collection and submission of ACP outcomes per national guidelines.
