Job summary

National Cancer Centre Singapore (NCCS) is seeking a Senior Executive / Assistant Manager to support right-siting and shared care for cancer survivors under the Healthier SG framework. The role focuses on patient education, care coordination, and stakeholder engagement to ensure continuity of care as patients transition from specialist care to primary care providers.

Qualifications

• Bachelor’s degree in any discipline.
• Experience in healthcare or administrative roles preferred.
• Proficient in Microsoft 365.
• Excellent written and verbal communication.
• Strong interpersonal and patient-oriented approach.
• Self-motivated and able to work independently.

Responsibilities

• Counsel patients on right-siting and ACP.
• Educate patients on follow-up with Primary Care Providers (PCPs).
• Coordinate right-siting appointments and referrals.
• Prepare documents (e.g., Cancer Survivorship Care Plan) for right siting and shared care.
• Maintain patient records, right-siting, shared care and ACP database.
• Coordinate reminders and follow up on first PCP visit and surveillance tests.
• Facilitate referrals back to NCCS when required.
• Support quality improvement and expansion of the programme.
• Liaise with internal and external stakeholders on programme implementation.
• Provide administrative support and perform other duties.
