Extendicare is seeking a self-directed Nurse Practitioner (NP) to deliver comprehensive primary health care for long‑term care residents in Ontario. This full‑time role emphasizes leadership within an inter‑professional team to support knowledge development and quality improvement.

You will collaborate with administrators, medical directors and care teams, while providing direct resident care and education to staff and learners, including residents' families.

Qualifikationen

• Ontario RN with Extended Class NP registration.
• Master’s degree in Nursing is an asset.
• Active professional liability insurance recommended.
• Registration with NPAO preferred.

• Provide primary health care to LTC residents with acute and chronic conditions.
• Lead LTC team with education, quality improvement, and program development.
• Collaborate with physicians, staff and families to create person‑centered care plans.
• Perform comprehensive assessments, orders, and evidence‑based management.
