Job summary

Johns Hopkins Aramco Healthcare in Saudi Arabia is seeking a skilled Case Manager to lead and participate in clinical pathway projects, data analysis and quality improvement. You will mentor staff, screen patients, and coordinate multidisciplinary plans to optimize discharge planning and resource use.

Required are a Bachelor of Nursing, at least 4 years of nursing experience post-licensure, and strong communication, project management and organizational skills.

Qualifications

• Bachelor's Degree in Nursing is required.
• Minimum of 4 years of applicable nursing experience after licensure.
• Strong Oral and Written Communication Skills, Project Management, Quality Improvement, Change Management, Time Management and Organizational skills are required.

Responsibilities

• Actively participate in development and implementation of Case Management projects including Clinical Pathways taskforce committees, quality improvement tools, data analysis and variance reports.
• Maintain professional development as a Case Manager, staying updated with clinical changes and evidence-based practice.
• Mentor and orientate new or less senior staff about case management roles and responsibilities.
• Complete initial screening of all assigned patients to identify strengths and needs related to clinical resource utilization and discharge planning.
• Collaborate with healthcare team to ensure the multidisciplinary plan of care is developed, followed and modified as needed.
• Identify variances from plan of care and promote resolutions for patient, family and carer; monitor multidisciplinary care activities and develop delivery strategies.
• Determine medical necessity of hospital stay and refer cases not meeting criteria as directed.
• Assist outpatients in setting SMART goals and teaching self-management; report abnormal findings to the physician team.
• Participate in process improvement activities related to case management and integrate research findings into practice.
• Represent care management in committee meetings such as long stay reviews and clinical guideline development.

• Collaborate with members of the healthcare team to ensure the multidisciplinary plan of care is developed followed and modified as needed
• Identify variances from the plan of care and promotes resolutions for the patient family and carer monitor multidisciplinary patient care activities to develop appropriate care delivery strategies for identified patients and make recommendations as required Ensure that interventions are aligned with the medical management initiatives
• Use established criteria to determine the appropriateness of the admission and to determine medical necessity of hospital stay forinpatients refer cases not meeting admission or continued stay criteria to the physician reviewer and appropriate administrative staff as directed
• Assisting outPatients in setting specific measurable achievable realistic and timebound SMART goals for selfmanagement teaching them how to do selfmanagement tasks and reporting abnormal findings to their physician team
• Participate in process improvement activities related to case management practice and incorporates the research process into clinical practice through communication application evaluation of research findings as well as through participation in research activities relevant to area of expertise
• Participate and represent care management in relevant committee meetings eg long stay review Clinical Guideline Development Length of stay reduction work groups and staff meetings

Bachelor's Degree in Nursing required.

Professional Certifications Required

Current licensure as a professional registered nurse. 3 years minimum clinical nursing experience following licensure/registration in the country of origin, to include 2 years Case Manager/case coordinator experience preferred.Demonstrated strong Oral and Written Communication Skills, Project Management, Quality Improvement, Change management, Time Management and Organizational skills are required.
