Job summary

Spring Health is hiring a full-time, remote SUD Specialty Care Navigator to independently manage member care with licensure and advanced skills. You will triage, guide, and coordinate services, applying evidence-based practices to reduce substance use and support recovery.

Schedule: Monday–Friday in ET, with responsibilities spanning triage, coordination, and collaboration with multidisciplinary teams. Excellent communication and crisis intervention skills are essential, along with compliance

Qualifications

• Mental Health Licensure required: LCSW, LMFT, LPC with 2+ years of experience in substance use disorder (SUD) clinical care.
• Proficiency in motivational interviewing, triage, ASAM criteria and various levels of care and best practices for SUD.
• Excellent listening, written, and verbal communication skills.
• Ability to work effectively with diverse populations and a commitment to inclusion.

Responsibilities

• Conduct outbound calls, triage calls, and follow-up with individuals seeking SUD treatment.
• Engage with members using advanced clinical techniques to assess needs and advise on treatment options.
• Coordinate appointments, referrals, and follow-ups, ensuring high-quality care.
• Provide psycho-education and support to help members achieve recovery goals.
• Utilize knowledge of ASAM levels of care and SUD treatments to guide recommendations.
• Offer crisis intervention and safety planning for high-risk situations.
• Maintain detailed records in EHR and other systems.
