Job summary

Maryland Care Management, Inc. (MCMI) seeks a registered nurse to lead the planning, development, and oversight of care plans for high-risk members. The role emphasizes coordination of care, quality outcomes, and utilization management within a multidisciplinary team.

The position requires 4-5 days/week onsite at Meritus in Hagerstown, collaborating with physicians and care teams to ensure timely assessments and optimal patient care while maintaining HIPAA compliance.

Qualifications

• Registered Nurse with current state license.
• 2+ years of clinical nursing experience in acute care or community setting.
• 2 years of care management experience.
• Knowledge of Managed Care Programs (preferred).
• BSN (preferred).
• CCM (preferred).

Responsibilities

• Develop, maintain, and monitor processes for timely identification of high risk and special needs members.
• Collaborate with physicians and interdisciplinary teams to determine medical needs.
• Conduct interviews to identify risk factors and care management needs.
• Foster networks with service providers to ensure quality care continuity.
• Plan and implement care plans to deliver high-quality care and service.
• Discharge members from care management per policy and procedures.
• Maintain care management records and ensure data integrity in health systems.
• Collect, evaluate, and report clinical and program activity.
• Educate members' providers to address unique patient needs in planning.
• Coordinate with community support and social services for members.
• Contribute to policy/procedure development for care management.
• Communicate with supervisor on departmental and program status.
