Job summary

Maryland Care Management, Inc. (MCMI) is hiring a Care Coordinator to support the CM team with short-term care coordination, education, and access to services for members.

Under supervision, you will screen eligibility, collect data for HRAs, coordinate with PCPs and providers, and help members navigate medical homes and social services to improve health outcomes.

Qualifications

  • High school diploma or equivalent.
  • Two or more years of experience serving the Medicaid population preferred.
  • Valid State Driver’s License required.
  • Knowledge of community resources and social services helpful.

Summary/Position Objectives:

Under direct supervision, works closely with care management (CM) team to provide short-term care coordination and connection to resources. The Care Coordinator collaborates with members, providers, and the CM team to support members’ health and overall well-being through education, care coordination, and access to services.

What This Position Does:
  • Provides administrative support to the members of the CM team.
  • Collects data for Health Risk Assessments (HRA)
  • Screens for eligibility and benefits.
  • Identifies members without a PCP and refer to Member Services.
  • Screens members by priority for CM services and refer as appropriate.
  • Performs transition of care duties to include, but not limited to, contacting the members’ PCP, Medical POA, or other medical providers (i.e., specialists, home health agencies) for information pertaining to special needs.
  • Produce and mail routine CM letters and program educational materials.
  • Documents all encounters and contacts made on behalf of clients; completes and submits monthly reports; maintains comprehensive electronic member records, which include notes, release of information, assessments and other medical documents acquired on behalf of the member.
  • Motivates members to be active and engaged participants in their health and overall well-being.
  • Provides educational material and member follow-up, arrange PCP visits, and perform care coordination under the direction of the Care Manager.
  • Assists members in accessing health related services, including but not limited to obtaining a medical home, understanding the appropriate use of the medical home, overcoming barriers to obtaining needed medical care and/or connecting with social services.
  • Proactively assist with care coordination of needed resources to address barriers or SDoH needs, close gaps in care, and help with the appropriate access to care.
  • Follows up with both members and providers regarding identified health and/or social services.
  • Perform tasks as directed by the CM team to promote member compliance such as verifying appointments or obtaining medical records.
  • Continuously expand knowledge and understanding of community resources and services.
  • Facilitates member access to community resources, including, but not limited to, housing, food, clothing, prenatal and parenting classes, life-skills education, and relevant mental health services.
  • Facilitates communication and coordinate services between providers and the members.
  • Work collaboratively and effectively within a team. Establishes positive, supportive relationships with participants and provide feedback to other members of the team. Build and maintain positive working relationships with members, providers, nurse care managers, social workers, agency representatives, and office staff, from diverse cultural and socio-economic backgrounds.
Secondary Functions:
  • Maintain the confidentiality and security of member records in accordance with MPC policies and procedures and applicable privacy requirements.
  • Possession of a valid State Driver’s License is a requirement for this position.
Requirements
Knowledge and Skills:
  • Strong interpersonal, communication, and customer service skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Knowledge of community agencies and resources.
  • Knowledge of medical terminology.
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.
  • Knowledge of transportation and other barriers to care that members may face.
  • Ability to communicate medical information to health care professionals and members of the care management team.
  • Creative and analytical thinking; solid written and oral skills.
  • Strong problem-solving, organizational, and time management skills.
  • Proficiency with computers and Microsoft Office applications preferred.
  • Understanding of remote work tools and platforms (Microsoft Teams, Zoom, SharePoint, etc.)
Education and Work Experience:
  • High school diploma or equivalent
  • Two or more years of experience serving the Medicaid population in the Maryland or Washington, D.C. area preferred.
