Job summary

Maryland Care Management, Inc. is seeking a Care Manager to plan, develop, implement, and oversee care plans for high-risk members with identified needs.

This role focuses on improving coordination, continuity, and appropriate utilization of health care services and resources. You will collaborate with physicians and an interdisciplinary team, conduct assessments, and build relationships with community providers to ensure quality care while managing costs.

Qualifications

  • Graduate of an Accredited School of Nursing.
  • Current, unrestricted RN license in Maryland.
  • 2+ years of clinical nursing experience in an acute care or community setting.
  • 2+ years of Care Management experience.
  • BSN preferred and CCM preferred.
  • Experience assisting members with accessing community agencies and resources.

Responsibilities

  • Develops, maintains, and monitors processes to identify high risk and special needs members.
  • Acts as an interdisciplinary team member in member management with physicians.
  • Conducts telephone or face-to-face interviews to identify risk factors for care management.
  • Fosters networks with providers to promote continuity and quality of care.
  • Develops and implements care plans to deliver quality care and service.
  • Maintains care management records and ensures data integrity.
  • Educates medical providers regarding members' special needs.
  • Coordinates with community support and social services.
  • Reduces care costs while maintaining quality.
  • Collaborates with teams to remove care management barriers.
  • Develops policies and procedures related to care management.
  • Communicates with supervisor regarding departmental issues.

Summary/Position Objectives:

The Care Manager plans, develops, implements, and oversees care plans for members with identified care management needs. This role improves the coordination, continuity, accessibility, and appropriate utilization of health care services and community resources for high-risk members and members with special needs.

What You'll Do:
  • Assists in developing, maintaining, and monitoring processes to promote the timely identification and assessment of high risk and special needs members.
  • Serves as an interdisciplinary team member in management of identified members, performing clinical/social determinants of health assessments and in collaboration with physicians, determining medical needs.
  • Conducts telephone or face-to-face interviews of members to identify risk factors and the need for care management.
  • Fosters professional networks and relationships with service and resource providers to promote continuity and quality of care for members.
  • Initiates, develops, plans, and implements care plans designed to promote the delivery of quality care and service to members with identified CM needs.
  • Utilizes clinical judgment to discharge members from care management according to plan policies and procedures.
  • Maintains care management records according to plan policies and procedures and ensures data integrity in health plan information systems.
  • Collects, evaluates, and reports clinical, functional and program activity as applicable.
  • Identifies cases to assess for impact of care management services, and assists with data collection as needed.
  • Educates members' medical providers regarding the members' special needs to ensure consideration to the unique needs of the patients in treatment planning.
  • Coordinates and communicates with community support and social service systems for members.
  • Reduces care costs without sacrificing quality through effective service coordination and multidisciplinary collaboration.
  • Collaborates with multidisciplinary teams to facilitate member care and reduce care management barriers.
  • Assists with the development of policies and procedures related to care management.
  • Communicates effectively with supervisor/manager regarding departmental issues, member issues, program status, compliance issues, etc.
Secondary Functions:
  • Following HIPAA guidelines, the Care Manager maintains confidentiality of patient information and adheres to appropriate release of medical information procedures.
  • Demonstrates an understanding of the functions of other departments, communicates with other departments appropriately and maintains positive working relationships.
  • Identifies and reports gaps in the service delivery system.
Knowledge and Skills:
  • Strong computer and keyboarding skills with proficiency in Microsoft Office 365 and common computer and office hardware.
  • Understanding of remote work tools and platforms (Microsoft Teams, Zoom, SharePoint, etc.)
  • Ability to perform mathematical skills and statistical calculations accurately.
  • Demonstrated written and verbal communication skills.
  • Ability to conduct presentations and training before small and large groups.
  • Knowledge of the Americans with Disabilities Act of 1990.
  • Strong problem-solving, organizational, and time management skills with the ability to consistently work in a fast-paced environment.
Education and Work Experience:
  • Graduate of an Accredited School of Nursing.
  • Current, unrestricted Registered Nurse license to practice in Maryland.
  • 2+ years of clinical nursing experience in an acute care or community setting.
  • 2+ years of Care Management experience.
  • Experience assisting members with accessing community agencies and resources.
  • Knowledge of government-sponsored managed care programs is preferred.
  • BSN preferred.
  • CCM preferred.
