Manila Doctors Hospital is seeking a pharmacist to manage outpatient and inpatient dispensing, unit-dose dispensing, oncology pharmacy, and satellite operations across operating rooms, delivery rooms, and emergency departments.
You will verify, prepare, dispense, counsel patients, monitor patient drug profiles, manage inventory, and collaborate with other healthcare practitioners to ensure safe, effective medication use.
About the role
This is a pharmacy role involving outpatient and inpatient dispensing, unit dose dispensing, oncology pharmacy, and satellite pharmacy operations across operating rooms, delivery rooms, and emergency departments. The role encompasses medication verification, preparation, dispensing, patient counseling, inventory management, and coordination with other healthcare practitioners.
Key responsibilities
Verify, prepare, dispense and charge drug products in accordance with established policies, procedures and pertinent statutory and regulatory requirements
Provide written and verbal counseling concerning medication use and other drug information suitable to patient needs
Assure completeness and interpret appropriateness of medication orders including age-related factors before preparing or permitting distribution of drug products
Maintain accuracy of patient drug profiles
Prepare and label medication requests using appropriate technique and following health system policies and procedures
Ensure policies and procedures related to drug distribution are followed
Compound preparations not available commercially such as compounded prescriptions, Total Parenteral Nutrition (TPN) and other I.V. admixtures
Display initiative in preventing, identification and reporting pharmacy-related patient care problems and medication errors
Check daily census and admission status of patients and review medication orders for newly ordered, discontinued, shifted, or adjusted medications
Notify physicians of therapeutic incompatibility, potential drug interactions and duplicate prescriptions
About you
Ability to verify, prepare and dispense drug products in accordance with established policies and procedures
Knowledge of medication use, drug information and patient counseling
Ability to interpret appropriateness of medication orders including age-related factors
Competency in preparing and labeling medication requests using appropriate technique
Ability to compound preparations including compounded prescriptions, Total Parenteral Nutrition (TPN) and I.V. admixtures
Skill in identifying and reporting pharmacy-related patient care problems and medication errors
Ability to provide drug information and respond to pharmacy-related queries
Competency in preparing antineoplastic IV admixtures and boluses using standard antiseptic technique
Ability to conduct inventory monitoring and management of base stock and emergency-cart medicines
Commitment to confidentiality of records and adherence to data privacy law and hospital data privacy policy
