Job description

Overview

The Pharmacist dispenses drugs prescribed by physicians and other health practitioners using the appropriate technique and following the health system’s policies and procedures. He/she assures the distribution and availability of supply of medicines for inpatients and provides quality pharmaceutical care to all patient care areas including OR, DR, ER, and Medical Ward.

Duties and Responsibilities

1. Fills prescriptions and dispenses pharmaceutical products.
2. Review prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability.
3. Labels all medicine containers issued to various services.
4. Checks the manufacture and expiration dates of the drug products and the label of medicines in accordance with BFAD labeling requirements.
5. Records issuances of medicines and accomplishes issue report forms after every shift.
6. Analyzes prescribing trends to monitor patient compliance and prevent excessive usage or harmful interactions.
7. Notifies physicians of any therapeutic incompatibilities, potential drug interactions, and duplicate prescriptions.
8. Ensures that the right patient takes the right drug and the right dosage at the right time.
9. Conducts daily/weekly/monthly inventory of medicines and supplies.
10. Prepares, checks, and signs charge slips issued to patients.
11. Collaborates with other healthcare professionals to plan, monitor, review, or evaluate the quality of effectiveness of drugs and drug regimens, providing advice on drug applications or characteristics.
12. Attends scientific seminar-workshops/consultative meetings and continuing education on drug-related matters and hospital pharmacy practices.
