Job summary

MATTGELO CORPORATION in the Philippines is seeking a dedicated social worker to assess clients’ social, emotional, and behavioral needs and develop individualized care plans. You will provide counseling, advocacy, and coordination of services with other professionals to support clients and families.

You will monitor progress, maintain confidential records, and participate in team meetings and community outreach while staying informed about best practices and relevant regulations.

Responsibilities

• Conduct assessments to evaluate clients' social, emotional, and behavioral needs.
• Develop individualized care plans and intervention strategies to address clients' challenges.
• Provide counseling, therapy, and support services to clients dealing with various issues such as mental health, substance abuse, domestic violence, etc.
• Advocate for clients' rights and access to services within the organization and the community.
• Collaborate with other professionals, agencies, and organizations to coordinate services and referrals for clients.
• Monitor and evaluate clients' progress and make adjustments to intervention plans as needed.
• Maintain accurate and confidential client records and documentation.
• Educate clients and their families about available resources, coping strategies, and self-care techniques.
• Participate in case conferences, team meetings, and community outreach activities.
• Stay informed about social work best practices, ethical standards, and relevant laws and regulations.
