Job summary

NUEVA ECIJA EYE AND LASER CENTER, INC is seeking a Social Worker to support patients through social and financial assessments, identify indigent patients, and assist with completing necessary documents and applications.

The role also provides psychosocial counseling, maintains confidential records, prepares case reports, and participates in community eye care programs, reporting to management as needed.

• Conduct social and financial assessment of patients.
• Identify and validate indigent and financially disadvantaged patients.
• Assist patients in completing required documents and applications.
• Provide basic psychosocial support and counseling to patients and families.
• Maintain accurate and confidential patient records.
• Prepare social case assessments and required reports.
• Participate in community outreach and eye care programs.
• Perform other related duties assigned by management.
