• Provide discharge planning assistance for inpatients and outpatients as deemed necessary and as requested.
• Coordinate coverage to ensure that continuity of care needs of all patients is met in all service areas.
• Prepares referral slip to PCSO.
• Perform social casework functions in patients with social emotional and/or environmental problems that may affect their medical situation through purposeful interview with them and their families.
• Perform eligibility study on patients with social and financial capabilities.
• Maintains quality results by following standards.
• Protect hospital’s value by keeping collection information confidential.
• Perform such other duties and responsibilities analogous or related to and or implied from the above enumeration of duties and responsibilities
