Qualifications

Responsibilities

• Receive, review, and process patient referrals from approved healthcare sources.
• Enter and update patient and referral information accurately in company systems.
• Verify required documents, insurance eligibility, authorizations, and referral requirements.
• Follow up on missing information and monitor referrals until completion.
• Communicate with patients, caregivers, healthcare providers, and internal teams regarding referral status and next steps.
• Schedule assessments, onboarding calls, and other required appointments.
• Prepare intake documentation and coordinate timely handoffs to the appropriate teams.
• Escalate urgent referrals, eligibility concerns, documentation gaps, and patient-safety concerns.
• Maintain accurate and confidential records in compliance with HIPAA and company policies.
• Support quality and process improvement initiatives.
