Job summary

Responsibilities

• Provides education and emotional support to patients and helps guide them through the healthcare system.
• Acts as a liaison with referring physicians, patients and caregivers to provide resources and assistance with accessing supportive care services.
• Assesses patients for referrals to resources and coordinates logistics for plan adherence such as appointment reminders and transportation.
• Assists patients with applying for insurance and communicates insurance information to care teams.
• Documents patient care, tracks outcomes and participates in care planning meetings for discharge or ongoing care.
