Amae Health is seeking a Clinical Care Coordinator to support members in daily experiences, facilitate group activities, and help maintain accurate records within our interdisciplinary clinic teams.

You will collaborate with psychiatrists, therapists, dietitians, and primary care providers, ensuring smooth operations, effective communications, and growth in member engagement and access to resources.

Qualifications

• Enrolled or recently graduated in a degree related to mental health such as pre‑med, psychology, social work, or nursing.
• Strong interpersonal skills and ability to work in a team environment.
• Detail-oriented and organized with good record-keeping.

Responsabilités

• Facilitate groups, arts and crafts, and recreational activities to enhance member experience.
• Welcome members, manage communications via phone and email, assist with scheduling and records.
• Collaborate with psychiatrists, therapists, dietitians, and primary care providers for coordinated care.
• Identify members’ needs and connect them with community resources and supports.
• Contribute to clinic growth and participate in training to build mental health support skills.
