Job summary

Hammerjack Pty Ltd is seeking two skilled Assistant Case Managers to support the Case Management team with medical records management, documentation, and provider communication. You will request, track, retrieve, organize, and upload medical records while ensuring timely receipt and accurate documentation for legal, insurance, and clinical purposes.

Qualifications

• Experience with Filevine or similar case management systems is required.
• Strong data entry and meticulous record-keeping capabilities.
• Experience communicating professionally with healthcare providers, clients, and team members.
• Ability to manage multiple medical record requests and strict confidentiality.

Responsibilities

• Request, track, follow up on, and retrieve medical records from providers.
• Digitize, organize, and upload records into client case files with accuracy.
• Monitor multiple requests and maintain detailed tracking logs.
• Generate weekly progress reports for the Case Manager.
• Communicate professionally with healthcare providers, clients, and internal staff.
• Maintain organized case files in Filevine and other systems.
• Ensure confidentiality of sensitive client and medical information.
• Follow up to ensure timely completion of records.
