Medical Coder - Inpatient DRG (IPDRG)

Responsibilities

• Analyze physician/provider documentation in inpatient health records to determine principal/secondary diagnoses, principal/secondary procedures, and assign appropriate ICD‑10‑CM, ICD‑10‑PCS codes.
• Assign Medicare Severity Diagnosis‑Related Groups (MS‑DRG), All Patient Refined DRGs (APR), Present on Admission (POA), Severity of Illness (SOI), and Risk of Mortality (ROM) indicators.
• Identify Hospital Acquired Conditions (HAC) and Patient Safety Indicators (PSI) to ensure accurate reimbursement.
• Use coding guidelines from CDC, CMS, AHA, AHIMA, and company procedures.
• Navigate patient records and electronic systems to determine accurate codes.
• Employ encoder software and online tools for ICD coding, MS‑DRG, APR, POA, SOI, ROM.
• Review inpatient records for clinical evidence to support diagnosis, MS‑DRG, APR assignments and reduce denials.
• Code consecutive/combined accounts complying with the 72‑hour rule and other combine scenarios.
• Maintain inpatient coding quality and productivity standards.
• Protect patient data per HIPAA standards.
• Manage coder queries: consult physicians, Clinical Documentation Specialists, or other providers to obtain missing documentation.
• Track and follow up on EMR queries using communication tools.
• Collaborate with HIM and Patient Financial Services teams to resolve billing, claims, denial, and appeals issues.
• Maintain Continuing Education Units (CEUs) for coding credentials.
• Stay current on inpatient coding and reimbursement guideline changes.
• Report coding or EMR workflow issues to supervisors.
• Illustrate awareness of documentation or coding ethics concerns.
• Maintain knowledge of applicable federal, state, and local coding regulations and Code of Ethics.
• Perform abstracting of additional data elements.

Required Qualifications

• Minimum 2 years of inpatient DRG coding experience.
• Bachelor of Science in allied health or related field.
• Active Certified Coding Specialist (CCS), Certified Inpatient Coder (CIC), or Certified Documentation Improvement Practitioner (CDIP).
• Extensive knowledge of medical terminology, anatomy, diagnostic, and procedural coding; proficiency in MS‑DRG, APR assignment; POA, SOI, ROM indicators; HAC, PSI identification; HIPAA familiarity.
• Proficient with electronic medical records (Epic, Cerner, Meditech).
• Excellent verbal and written English, CEFR level B2 or higher, strong customer service skills.

Preferred Skills/Expertise

• Master’s degree or credential in business, healthcare, or related field.
• Registered Nurse with active PHRN or USRN license.
• Experience with encoding/grouping software and Computer Assisted Coding (CAC) – 3M, Solventum, Encoder Pro, Codify.
• Proficiency with Microsoft Office suite, including Excel, Word, PowerPoint, Outlook, SharePoint.
• Prior experience with US healthcare providers or payers.
