Job summary

Community Health Resources, Inc. (CHR) is seeking a Housing Support Specialist II to provide direct case management and housing stabilization services for individuals experiencing homelessness or risk of housing instability in the HERO program region 4.

The position requires a bachelor's degree and a valid driver’s license, with experience working with homeless populations. The role includes collaboration with community partners, managing housing applications, and advocating for tenant rights.

Qualifications

• Education: Bachelor’s Degree required, Human Services preferred.
• Experience: 2 years’ experience with homeless population or related groups.
• Licensure/Certification/Registration: Valid driver’s license.

Responsibilities

• Provide direct case management, develop housing stabilization plans, and assist with benefits, budgeting, and services.
• Assist clients with applications for Husky, Social Security, and other entitlements.
• Help clients access job and vocational training opportunities.
• Link clients to mental health or substance use counseling and DCF services.
• Provide security deposits and time-limited rental assistance as needed.
• Advocate with landlords for tenant rights and resolve conflicts.
• Interface with CHR, homeless service providers, and related agencies to coordinate access.
