Responsibilities

• Coordinate scheduling and deployment of home care nurses, therapists, and other healthcare providers.
• Communicate with patients and families regarding service schedules and care plans.
• Serve as a primary point of contact for patients and families regarding home care appointments, service concerns, and care updates.
• Monitor daily operations to ensure smooth delivery of services and adherence to hospital standards and regulatory guidelines.
• Collaborate with physicians, nurses, and hospital departments to ensure coordinated and continuous patient care.
• Promote patient safety and support all related programs and policies to ensure delivery of excellent patient care.
