Responsibilities:
- Provide administrative support to patients and staff in alignment with Care Coordination team care plans.
- Assist clinical and administrative staff with scheduling patient appointments, connecting patients to appropriate resources, and conducting follow-up tasks as delegated by the Care Manager.
- Serve as a resource for patients and families, offering assistance based on Care Coordination protocols.
- Maintain accurate and comprehensive progress notes for every patient interaction.
- Build and sustain strong working relationships with community service providers and stay informed about available community resources.
- Manage required data, documentation, and reports for the Care Coordination team.
- Assign Care Managers to referred patients, support initial and follow-up scheduling, send necessary written communication to patients, confirm services, and elevate clinical concerns to the Care Manager when needed.
- Support patients in navigating insurance requirements, including prior authorizations and out-of-network waivers.
Must Have Qualifications:
- High school diploma (required)
- Associate’s degree in healthcare or related field (preferred)
- At least 1 year of experience in a community or ambulatory care setting
Preferred Qualifications:
- Experience in a healthcare environment working with patients, families, and providers
- Background in case management within ambulatory or transitional care settings
- Ability to interpret and synthesize patient care plans and resource needs
- Experience using EMR systems
- Strong customer service skills with the ability to enhance patient experience while meeting program goals
- Excellent organizational skills with the ability to present information clearly and concisely
- Effective interpersonal and communication skills for collaborating with physicians, patients, families, and interdisciplinary team members
