Job summary

Qualifications

• Bachelor’s degree in Business, Healthcare, or related field required.
• At least 1 year of experience in a community, ambulatory care, or healthcare-based setting required.
• Experience working with patients, families, and healthcare providers required.

Responsibilities

• Provide administrative assistance to patients and Care Coordination staff in supporting patient care plans.
• Assist in scheduling patient appointments and coordinating care services as directed by Care Managers.
• Help connect patients and families with appropriate community resources and healthcare services.
• Maintain accurate and detailed progress notes for patient interactions.
• Prepare reports and maintain documentation required by the Care Coordination team.
• Assign Care Managers to referred patients and assist with initial and follow-up scheduling.
• Send written communication to patients and confirm service appointments as needed.
• Assist patients with navigating insurance requirements such as prior authorizations and out-of-network waivers.
• Escalate clinical concerns or decision-making requirements to Care Managers when necessary.
