The Patient Care Coordinator is the primary point of contact for patients, ensuring seamless coordination of care, managing inquiries, and maintaining accurate records. This critical role requires exceptional organizational and communication skills to facilitate efficient and compassionate service delivery. While prior healthcare experience is a plus, comprehensive paid training will be provided to ensure success.
The successful candidate will be responsible for, but not limited to, the following core functions:
- Communication Management: Handle high-volume inbound and outbound calls, emails, and internal communications related to patient care, appointments, and general inquiries.
- Appointment Scheduling: Coordinate and schedule patient appointments, procedures, and follow-up visits, ensuring optimal utilization of resources and timely care delivery.
- Documentation & Record Keeping: Accurately update and maintain patient records, ensuring all coordination activities are logged in compliance with data privacy standards.
- Liaison Services: Act as a bridge between patients, medical staff, insurance providers, and other external stakeholders to resolve service issues and facilitate authorizations.
- Service Excellence: Provide compassionate and professional customer support, addressing patient concerns and escalating complex medical or administrative issues to the appropriate personnel.
- Process Adherence: Strictly follow all organizational policies, protocols, and standard operating procedures (SOPs) learned during the paid training period.
