Qualifications

• Hands-on experience with Elation Health EMR and patient portal tools like Guava or similar onboarding platforms.
• Concierge, DPC, or high-touch practice experience where personalized service is essential.
• Proven background managing referrals, medical record transfers, and multi-provider care coordination.
• Excellent written and spoken English with professional email etiquette.
• Meticulous organization and ability to manage continuous email streams and logs.

Responsibilities

• Monitor and manage the central practice email inbox, ensuring prompt responses.
• Schedule consultations, routine check-ups, and follow-ups; coordinate with external physicians.
• Process, track, and confirm outgoing referrals and testing orders with notes for patients.
• Maintain patient charts in Elation Health and assist with Guava digital onboarding.
