Job summary

PM Consulting recruits a Care Coordination Support Specialist to provide administrative and patient support to the Care Coordination team in the Philippines. This role helps ensure efficient delivery of patient care services by assisting with scheduling, documentation, and resource coordination.

Qualifications

• Bachelor’s degree in Business, Healthcare, or related field is required.
• At least 1 year of experience in a community, ambulatory care, or healthcare setting.
• Experience working with patients, families and healthcare providers is required.
• Experience in case management in ambulatory or transition care settings is preferred.
• Previous experience in healthcare, BPO, or shared services environment is preferred.

Responsibilities

• Provide administrative assistance to patients and Care Coordination staff in supporting patient care plans.
• Assist in scheduling patient appointments and coordinating care services as directed by Care Managers.
• Help connect patients and families with appropriate community resources and healthcare services.
• Maintain accurate and detailed progress notes for patient interactions.
• Prepare reports and maintain documentation required by the Care Coordination team.
• Assign Care Managers to referred patients and assist with initial and follow-up scheduling.
• Send written communication to patients and confirm service appointments as needed.
• Assist patients with navigating insurance requirements such as prior authorizations and out-of-network waivers.
• Escalate clinical concerns or decision-making requirements to Care Managers when necessary.
