Job summary

North Bay Regional Center is seeking a Social Services Case Manager to join our dedicated team of Service Coordinators. You will manage the delivery of services to NBRC clients, including those with intensive needs, ensuring the Individual Program Plan and its objectives are pursued.

We require a Bachelor’s Degree in a related field; Master’s degree is preferred, with at least 3 years of experience working with individuals with developmental disabilities. Bilingual Spanish-English is preferred.

Qualifications

• Bachelor’s Degree in a field related to developmental disabilities (e.g. Social Work, Psychology, Counseling, Special Education, Nursing) is required or non-waivable.
• Master’s Degree preferred for advancement and leadership.
• Minimum 3 years’ experience working with individuals with developmental disabilities.
• Bilingual language ability (Spanish to English) preferred.

Responsibilities

• Use person-centered planning to assist clients in using public and private resources before regional center funds.
• Monitor vendor services and verify services align with purchase of service authorizations.
• Evaluate the effectiveness of clients’ Individual Program Plans and coordinate community resources.
• Conduct annual reassessments of clients’ developmental status with the interdisciplinary team.
• Conduct client assessments on an occasional basis.
