Job summary

The Fono is seeking an Outreach Manager to lead a multidisciplinary team across Auckland sites (Henderson, Blockhouse Bay or Manurewa). You will ensure services are culturally responsive, accessible and equity-driven, working with staff, patients, whānau and partners to improve health outcomes for Pacific and Māori communities.

You will be responsible for coaching staff, meeting KPIs, and strengthening systems and reporting while promoting patient-centred care and strong community links.

Qualifications

  • You are a confident people leader with a passion for community wellbeing.
  • You bring experience leading multidisciplinary teams and a strong understanding of primary healthcare.
  • You understand Pacific and Māori health equity issues and improving access to care.

Job description

Reporting to the Outreach Manager, you will provide day-to-day leadership, professional support and operational oversight to a multidisciplinary team including Registered Nurses, Health Coaches, Kaiāwhina and Outreach Administrators.

You will play a key role in ensuring our outreach services are culturally responsive, community-focused, and achieve meaningful outcomes for the people we serve.

Based at one of our Auckland sites (Henderson, Blockhouse Bay or Manurewa), this role involves working closely with staff, patients, whānau, community partners and healthcare providers to ensure services are accessible, effective and aligned with The Fono's commitment to equity.

  • Leading and supporting a high-performing outreach team.

What You Will Bring

You are a confident people leader who is passionate about community wellbeing and making a positive impact.

You will bring:

  • A relevant qualification in health, social services, community development or a related field.
  • Experience leading multidisciplinary teams.
  • Strong understanding of primary healthcare, outreach services and community-based support.
  • Knowledge of Pacific and Māori health equity issues and the factors influencing access to care.
  • Proven ability to support staff performance, wellbeing and professional development.
  • Excellent organisational, communication and relationship management skills.
  • A commitment to culturally responsive and equity-focused service delivery.
  • A current full New Zealand Driver Licence.
