You are a NZ Registered Nurse with at least 3 years' experience in primary care or community nursing, hold an annual practising certificate, and have competence in health equity, wound care, and chronic disease management.

As an Outreach Nurse, you will provide primary healthcare across a range of community settings, including patients' homes, community venues, outreach clinics, mobile medical units and The Fono clinics.
You’ll work alongside an integrated team of Registered Nurses, GPs, Social Workers, Kaiāwhina, Health Coaches and other health professionals to provide holistic, whānau-centred care.
You’ll play an important role in reaching people who may otherwise struggle to access healthcare, helping to remove barriers, improve health literacy, support prevention and ensure patients receive the ongoing care they need.
Most importantly, you will have the opportunity to build genuine relationships with patients and whānau and see the difference your work makes.
