Qualifications

• Must hold New Zealand work rights at the time of application.
• Experience in health, disability, rehabilitation, social services or related relationship-intensive work is advantageous.

Responsibilities

• Coordinate across health, disability and community services to arrange the right help.
• Bring people together to develop plans and navigate complex journeys.
