Job summary

Jora New Zealand is seeking an Outreach Nurse to provide primary healthcare across homes, clinics and outreach settings. You will work with a multidisciplinary team to deliver whānau-centred care and remove barriers to access.

The role requires a NZ Registered Nurse certificate, at least 3 years in primary healthcare, and strong clinical assessment, communication, and autonomous practice skills. CPR and a full NZ driver licence are essential.

Qualifications

  • Current NZ Registered Nurse Annual Practising Certificate.
  • At least 3 years' experience in primary healthcare or community nursing.
  • Strong clinical assessment and decision-making skills.
  • Ability to work confidently and autonomously in community settings.
  • Experience with diverse and/or high-needs populations.
  • Strong understanding of health equity and barriers to access to healthcare.
  • Excellent communication and relationship-building skills.
  • Current full NZ Driver Licence.
  • Current CPR certification.

Responsibilities

  • Deliver primary healthcare in homes, communities, outreach clinics and mobile settings.
  • Complete health assessments, clinical assessments and care planning.
  • Support patients with long-term conditions including diabetes and respiratory conditions.
  • Provide health promotion, education and preventative healthcare.
  • Support immunisation, screening, cardiovascular risk assessments and other health targets.
  • Provide wound care, ECGs, point-of-care testing and other nursing interventions.
  • Coordinate care between primary care, secondary services and community providers.
  • Follow up referrals and help patients stay connected with their healthcare.
  • Work alongside Kaiāwhina, Health Coaches and Social Workers to address health and social needs.
  • Advocate for patients and whānau who experience barriers to accessing healthcare.
  • Participate in community events, health promotion activities and mobile outreach services.
  • Contribute to quality improvement and better health outcomes for our communities.

As an Outreach Nurse, you will provide primary healthcare across a range of community settings, including patients' homes, community venues, outreach clinics, mobile medical units and The Fono clinics.

You'll work alongside an integrated team of Registered Nurses, GPs, Social Workers, Kaiāwhina, Health Coaches and other health professionals to provide holistic, whānau-centred care.

You'll play an important role in reaching people who may otherwise struggle to access healthcare, helping to remove barriers, improve health literacy, support prevention and ensure patients receive the ongoing care they need.

Most importantly, you will have the opportunity to build genuine relationships with patients and whānau and see the difference your work makes.

You will use your nursing expertise to:

What You Will Bring

We are looking for a Registered Nurse who is confident, compassionate and genuinely passionate about working with people and communities.

You'll bring:

  • A current New Zealand Registered Nurse Annual Practising Certificate.
  • At least 3 years' experience in primary healthcare, community nursing or a similar setting.
  • Strong clinical assessment and decision-making skills.
  • The ability to work confidently and autonomously in community settings.
  • Experience working with diverse and/or high-needs populations.
  • A strong understanding of health equity and the barriers that can prevent people from accessing healthcare.
  • Excellent communication and relationship-building skills.
  • A current full New Zealand Driver Licence.
  • Current CPR certification.
What Would Be Great to Have

It would be an advantage if you also have:

  • Authorised Vaccinator status.
  • Previous outreach or community nursing experience.
  • Experience working with Pacific and/or Māori communities.
  • The ability to speak a Pacific language and/or Te Reo Māori.
  • Experience using Medtech or other clinical information systems.
