Job summary

Access Community Health in Christchurch is seeking a CREST Support Worker to deliver rehabilitation support to tangata in their own homes. You will work with a multidisciplinary team to help people transition safely from hospital back into the community, supporting recovery, independence and wellbeing.

Qualifications

• Hold a Level 3 or Level 4 Health and Wellbeing qualification or have equivalent, relevant experience
• Spinal injury
• Stoma Management
• Te Tiritio Waitangi and Bi-cultural approach
• Experience issuing prescribed medication
• Full/restricted driver’s license
• The ability to drive up to an hour to and from your tangata
• Be willing to start work at 7am
• Be available to work weekends, evenings and split shifts
• Provide flexibility to sleepover or wakeover
• Be willing to learn, reflect, and adapt your approach to what matters for each person
• Have experience in or exposure to caregiving, disability support, aged care, community work, or other people-facing roles
• Committed to culturally safe, respectful support for people from all backgrounds
• Intuitive and responsive to people’s needs

• Deliver rehabilitation programmes under the direction of Registered Nurses in people’s homes
• Provide safe, respectful, and person-centred support
• Support recovery, independence and wellbeing in the home environment
