Job summary

Placements24 is seeking a dedicated Community Health Worker to conduct outreach in Queenstown and surrounding areas. You will connect individuals with health services, promote preventative care, and support vulnerable community members.

Qualifications

• A qualification or certification in community health work or a related field is preferred.
• Previous experience in a community-based health or social care role.
• Excellent communication and interpersonal skills, with the ability to connect with diverse populations.
• Understanding of local health challenges and resources within the Queenstown area.
• Ability to work independently in the field and maintain accurate records.
• A valid driver's license and willingness to travel within the community.

Responsibilities

• Conduct door-to-door outreach to identify health needs within the community.
• Provide health education on topics such as nutrition, hygiene, disease prevention, and chronic illness management.
• Assist clients in registering for healthcare services and accessing medical appointments.
• Perform basic health screenings (e.g., blood pressure, blood sugar checks) and refer individuals as needed.
• Distribute health information materials and promote community health programs.
• Build trust and rapport with community members, acting as a liaison between them and healthcare providers.
