Job summary

Access Community Health is seeking a DN Scheduler to support the smooth coordination of District Nursing services in the community.

You will manage referrals, maintain accurate schedules, and assist the nursing team in meeting client timeframes, serving as a key link between the District Nursing team, referral centre, regional teams, clients, and external providers. This is a full-time role with Thursday–Monday work and optional home-based Saturdays and Sundays.

• Strong computer literacy and attention to detail.
• Ability to build positive working relationships with clinical teams, referrers, clients, and stakeholders.

Responsibilities

• Receive and allocate District Nursing referrals in line with service delivery timeframes.
• Maintain nurse schedules and update rosters as referral information, nurse feedback, or priorities change.
• Help coordinate coverage when absences or service changes may affect client visits.
• Keep referral, roster, and client information accurate across relevant systems.
• Communicate professionally with nurses, clients, ACC, GP practices, referrers, and internal teams.
• Escalate risks, issues, or rapid-response referrals to the appropriate nursing lead.
• Provide administration support where required, including referral closure and follow-up documentation.
