Job summary

Placements24 is seeking a highly experienced Senior Registered Nurse, specializing in Case Management, to join a remote healthcare team across New Zealand. You will coordinate patient care, advocate for patients, and collaborate with providers and insurers to navigate the system and optimize outcomes.

Responsibilities include developing care plans, coordinating services, monitoring progress, and ensuring smooth transitions of care, with a strong focus on telehealth and electronic health records.

Qualifications

• Must hold a current and unrestricted RN license.
• Minimum of 5 years in clinical nursing, with at least 2 years in case management or care coordination.
• Proficient with EHR systems and telehealth platforms.

Responsibilities

• Assess patient needs and develop individualized care plans.
• Coordinate care across providers and facilities.
• Advocate for patients' rights and preferences.
• Liaise with insurers to obtain authorizations and manage coverage.
• Monitor progress and adjust care plans as needed.
