Job summary

Access Community Health in New Zealand seeks an Access Community District Nurse to provide autonomous home‑based care. You will deliver comprehensive clinical services, support patients and whānau, and collaborate with a multidisciplinary team across community settings.

The role includes wound care, medication management (including diabetes), catheter care, and palliative/end‑of‑life support. Flexibility for a rotating roster, weekends, and on‑call duties is required.

Qualifications

  • Registered or enrolled with a current practising certificate.
  • Experience with long‑term conditions, aged care, or medical nursing desirable.
  • Experience in wound care, medication administration including diabetes and syringe drivers.
  • Catheter management experience beneficial.
  • Palliative/end‑of‑life care experience advantageous; willingness to upskill.
  • Ability to work autonomously with sound clinical judgement.

Responsibilities

  • Deliver clinical care in clients’ homes.
  • Build therapeutic relationships with patients and whānau.
  • Collaborate with a multidisciplinary team of nurses and allied health professionals.
  • Manage a busy caseload with good time management.
  • Participate in an on‑call roster and weekend/afternoon shifts as needed.

These are permanent full & part-time positions working across a rotating roster. Flexibility to work weekends and afternoon shifts, and participation in an on-call roster is essential

Ngā tohungatanga mō tēnei tūranga | Skills and experience
  • Registered or enrolled with a current practising certificate
  • Experience in long‑term conditions, aged residential care, or medical nursing highly desirable
  • Experience in complex and non-complex wound care, medication administration including diabetes & syringe drivers and catheter management
  • Palliative care end of life experience would be an advantage. A willingness to upskill is desirable.
  • Ability to work with a high degree of autonomy and sound clinical judgement
  • Strong organisational and time management skills to effectively manage and prioritise a busy caseload
  • A collaborative team player who builds strong professional relationships in the community
  • Confident with digital tools, apps, and computer systems
  • A knowledge of Christchurch an advantage
  • A current full New Zealand driver’s licence is essential
