Qualifications

• Registered or enrolled nurse with a current practising certificate.
• Experience in long-term conditions, aged residential care, or medical nursing is welcomed but not essential.
• Ability to plan and organise caseloads effectively, to establish priorities and meet deadlines in a demanding, client focused environment; ability to self-manage.
• A current full driver's license is essential.
• Digital confidence with using apps and computers.
• Demonstrated commitment to culturally safe practice, equity, and respectful engagement with Māori, Pacific and diverse communities.
• Strong communication skills that support safe engagement and health literacy considerations.
• Willingness to learn, reflect and adapt your practice in line with supervision, training, and continuous improvement.
• Work locally with the option to work from our Office.

Responsibilities

• Deliver specialist clinical care in people’s homes, shaping personalised support plans and high‑quality clinical care.
• Guide and collaborate with support workers to provide the best service.
• Work with a connected team of health professionals to achieve exceptional outcomes for clients.
• Provide culturally safe, respectful, and consent-based care and involve whānau/support networks as preferred by the person.
