Presbyterian Support Northern is seeking a Service Coordinator to coordinate person-centered home-supported care in Tauranga, combining clinical nursing skills with community-based service delivery.

You will work from the Enliven Tauranga office and travel to visit clients and lead Support Workers, liaising with families, allied health professionals and external partners to ensure timely, responsive services.

Qualifikationen

• NZ Registered nurse (essential).
• Ability to coordinate services in community health settings.
• Ability to work with diverse teams and stakeholders.

Aufgaben

• Coordinate services to ensure timely and appropriate support for service users.
• Undertake clinical assessments and reviews for non-complex clients.
• Develop and maintain individual support plans focused on independence and wellbeing.
• Support rostering, scheduling and staff–service user matching.
• Build relationships with NASC, DHBs, GPs and community partners.
• Contribute to reporting, quality systems and service improvement initiatives.
• Support staff development and promote a positive team culture.
• Honor Tangata Whenua and promote culturally appropriate practice.
