Job summary

Ngāti Hine Health Trust in Northland seeks a dedicated Support Worker to assist individuals in Residential services to lead meaningful daily lives, grow personally, and participate in community activities.

You will implement lifestyle plans, support decision making, coordinate with Whānau Ora teams, and help maintain safe, well-documented services while upholding Māori cultural values.

Qualifications

  • Have or be working towards a Certificate in Health and Wellbeing - Level 2. Training will be provided.
  • Hold a current First Aid Certificate.
  • Ability to work in a strong Maori cultural workplace and apply Maori values and practices.
  • Participate in planning Whanau Ora Lifestyle Plans and service objectives.

Responsibilities

  • Support residents in daily activities and personal care as directed by team leads.
  • Assist in developing Lifestyle plans identifying goals, abilities and resources.
  • Help residents realise goals and aspirations through lifestyle planning.
  • Ensure residents have control over choices and informed decisions.
  • Contribute to Service, Business and Strategic plans for Residential services.
  • Complete tasks on time as directed by team leaders and management.
  • Participate in house meetings and other required meetings in a cooperative manner.
  • Model professional behaviour reflecting the role and organisation.
  • Maintain a safe workplace through training, planning and ACC management.
  • Provide timely, appropriate and safe services including personal care and documentation.
  • Handle financial records with care and ensure compliance with NHHT standards.

POSITION PURPOSE & EXPECTED OUTCOMES

The purpose of this role is to provide support to people living within our Residential services to enjoy an everyday life that encompasses personal growth and development, strengthens wellbeing and independence and promotes integration within the community.

KEY DUTIES AND RESPONSIBILITIES

The specific duties and responsibilities include, but is not limited to:

  • Being responsible to the Kaitataki Whaikaha – Portfolio Lead Disability and Home Support Services and Whanau Ora Team Leader Residential and be able to follow their instructions implicitly.
  • Assist in developing and implementing quality Lifestyle plans with individuals that accurately identify choice, abilities, talents, resources and goals of each individual.
  • Support individuals to realize their goals, dreams and aspirations through the Lifestyle Planning process.
  • Ensure individuals have control over their own lives and have opportunity to make informed choices and decisions.
  • Assist the Kaitataki Whaikaha – Portfolio Lead Disability and Home Support Services, Management of NHHT and Whanau Ora Team Leader Residential to develop and implement Service, Business and Strategic plans for the Residential services
  • Ensure that all agreed objectives as directed by the Kaitataki Whaikaha – Portfolio Lead Disability and Home Support Service, Whanau Ora Team Leader Residential and or management of NHHT are satisfactorily completed on time.
  • Contribute to the house meetings and any other meeting as required, in an active, co-operative, collaborative, energetic and professional manner.
  • Set a good example for peers, colleagues, and all observers of the Trust’s activities.
  • Conduct oneself in a fully professional manner which constantly and positively reflects the nature of the role.
  • Ensure that the workplace is a safe environment through training, planning and ACC management.
  • Ensure that all work related tasks are performed efficiently and effectively at all times by providing timely, appropriate and safe services to individuals. This could include personal care, household chores, community activities and completion of all required documentation.
  • Ensure, when appropriate, that all financial responsibilities are carried out and performed with full due diligence and care and that all financial records, required by the Whanau Ora Team Leader Residential, comply with Ngati Hine Health Trust standards and are maintained fully and accurately at all times.
QUALIFICATIONS AND EXPERIENCE

The following qualifications, skills and experience are assessed as relevant for this position within the NHHT:

Qualifications
  • Have or be working towards a Certificate in Health and Wellbeing - Level 2. Training will be provided,
  • Hold a current First Aid Certificate
  • Has the ability and experience to work within a strong Maori cultural workplace and can display competence in the use and application of Maori cultural values and practices.
  • Can participate constructively in planning, including the development of Whanau Ora Lifestyle Plans, service objectives, other work plans and in the implementation of such plans.
  • Has proven ability to effectively deal with complex issues/emergencies and seek appropriate support when required.
  • Is able to meet the demands of rostered shift work
  • Has a proven ability to work un-supervised.
  • Fluency in Te Reo Māori training would be advantageous
Soft skills
  • The ability to mediate and resolve internal conflicts between employees
  • The ability to sow vision on new ways of doing things and bring people on a journey of transformation
  • The ability to empathise from the perspective of employees who work in challenging front-line situations or where employees are going through their own personal challenges
Technical skills
  • Hold a Full unencumbered New Zealand Drivers Licence.
  • Be an excellent communicator, skilled in both written and oral expression.
  • Be computer literate with experience in the use of Microsoft Office applications.
