Job summary

Access Community Health in Hamilton is seeking a proactive Care Coordinator to join our regional team. You will be the vital link between clients and Support Workers, ensuring high-quality, timely home-based care that enables independence and safety.

Responsibilities include coordinating client rosters, matching workers to needs, and maintaining accurate records while upholding privacy and organisational procedures. Your role supports safe care delivery and positive client outcomes.

Qualifications

  • Strong communication and relationship-building skills.
  • Excellent organisation, time-management, and ability to stay calm under pressure.
  • Solutions-focused approach with solid problem-solving and critical-thinking skills.
  • Confident computer user (Microsoft Office and general PC skills).
  • Experience in scheduling, coordination, customer service, or health preferred.
  • Knowledge of home-care, disability, health, or aged-care services is an advantage.
