Access Community Health in Hamilton, NZ, is seeking a proactive Care Coordinator to link clients with Support Workers and ensure safe, timely home care.

Qualifications

  • Strong communication and relationship‑building skills.
  • Excellent organisation, time‑management, and ability to stay calm under pressure.
  • Solutions‑focused approach with solid problem‑solving and critical‑thinking skills.
  • Confident computer user (Microsoft Office and general PC skills)
  • Experience in scheduling, coordination, customer service, or health preferred
  • Knowledge of home‑care, disability, health, or aged‑care services is an advantage

Responsibilities

We’re looking for a proactive and organised Care Coordinator to join our regional team. In this fast-paced role, you’ll be the vital link between our clients and Support Workers—ensuring high‑quality, timely care so people can live safely and independently at home.

What You’ll Do
  • Coordinate and adjust daily client rosters to ensure safe, timely care
