Access Community Health in Hamilton, NZ, is seeking a proactive Care Coordinator to link clients with Support Workers and ensure safe, timely home care.

You’ll manage daily rosters, match workers to client needs, and maintain records with privacy in mind. Strong communication, organization, and problem-solving are essential, along with experience in scheduling, customer service, or health roles.

Qualifications

• Strong communication and relationship‑building skills.
• Excellent organisation, time‑management, and ability to stay calm under pressure.
• Solutions‑focused approach with solid problem‑solving and critical‑thinking skills.
• Confident computer user (Microsoft Office and general PC skills)
• Experience in scheduling, coordination, customer service, or health preferred
• Knowledge of home‑care, disability, health, or aged‑care services is an advantage
