Job summary

Access Community Health l Uru Àtea is seeking an Advanced Support Worker to oversee how our support is delivered across the Hutt Region. The role includes working hands-on with tangata with complex needs and providing leadership to support workers.

You will liaise with tāngata, whānau and partner agencies to ensure care is well communicated and centred on what matters most to the person. The position involves working closely with a multidisciplinary team and supporting staff development.

Qualifications

• At least two years’ experience delivering disability, home-based or community support services.
• Must have a full New Zealand driver’s license.
• Proficient in using Microsoft Office; able to collaborate within multi-disciplinary teams.

Responsibilities

• Oversee how support is delivered across the Hutt Region.
• Lead on-the-job guidance and support to less experienced staff.
• Liaise with tāngata, whānau, colleagues and partner agencies to coordinate care.
• Champion client-centred care and ensure services align with what matters most to the person.
