Job summary

Qualifications

• Two years of professional clinical case management experience.
• Must be able to provide reliable transportation for site visits when required.
• Licensure requirements as per department (RN, LCSW, LMHC, PT, OT, etc.).

Responsibilities

• Coordinate case management from admission to post-discharge with physicians, social workers and team members.
• Monitor care plans and patient/family response, adjusting as needed.
• Negotiate with third-party payers regarding benefits, eligibility and utilization.
• Establish discharge planning goals and coordinate follow-up care.
