Job summary

University of Utah Health seeks an experienced RN Care Coordinator to enhance patient-centered care across the care continuum. This role coordinates patient care plans, supports bedside staff, and collaborates with the care team to address discharge barriers in a fast-paced 24/7 health system.

The candidate will participate in rounds, discharge planning, and post-discharge follow-up to maximize outcomes. A Bachelor's degree and active RN license in Utah are required, with an eCredential to be

Qualifications

Bachelor's degree in health care administration, health science or related field.
One year of related nursing experience.
Current RN license in Utah or eligible via interstate compact.
Current RQI eCredential within 30 days of hire.

Responsibilities

Assesses, communicates and addresses patient and family care needs throughout the patient's stay.
Acts as the key coordinator and contact for the patient care plan.
Acts as clinical resource and support for bedside staff to address complex patient needs.
Collaborates with other Care Coordinators and Charge Nurses to identify burden of care and assign core team members.
Participates in medical rounds at least two mornings a week.
Attends patient evaluation conferences twice weekly and updates on progress and discharge needs.
Updates and monitors nursing orders based on care plan changes.
May follow up with patients within one week of discharge.
