Enhabit Home Health & Hospice seeks a proactive professional to manage relationships with physicians, hospitals, and other referral sources to grow referrals and ensure smooth transitions to home-based care. You will educate partners about Enhabit programs and navigate post-acute care pathways to improve patient outcomes and satisfaction.

Qualifikationen

• Bachelor's degree in healthcare administration, marketing, or related field or a graduate of an approved school of nursing, therapy or social work; equivalent experience considered.
• Highly preferred graduate of an approved school of nursing, therapy or social work.
• Minimum two years of experience in healthcare business development, community relations, marketing, or sales.
• Experience with Medicare home health and hospice regulatory guidelines.
• Valid state driver's license and automobile liability insurance where required.
• Strong interpersonal, communication and presentation skills.

• Acts as primary relationship manager between Enhabit and referral sources such as physicians, hospitals, and health systems.
• Supports referral growth by educating about Enhabit services and program requirements to referral sources, patients, families, and community partners.
• Assists with post-acute care transitions to improve patient outcomes and satisfaction.
