Job summary

Andwell Health Partners is hiring a Hospice Admission RN to provide skilled nursing services in patients’ homes under physician orders, develop care plans, and educate patients and families. The role emphasizes patient rights, holistic care assessment, and symptom management in a community/home setting.

The position requires a Maine RN license, CPR, and at least one year of recent nursing experience, with opportunities for flexible scheduling and comprehensive benefits.

Qualifications

• Active RN license in Maine.
• CPR certification required.
• Minimum 1 year of recent clinical nursing experience.
• ANA/Home Health or Community Health Nursing certification encouraged.

Responsibilities

• Provide skilled nursing services to the patient in their own home under the direction of a physician’s plan of care.
• Develop plans of care with realistic/measurable outcomes in collaboration with the patient/family.
• Provide education and promote patient’s rights and responsibilities for informed decision-making in the plan of care.
• Assess and identify the patient’s holistic health care needs.
• Demonstrate competency in assessment and technical skills.
• Possess and utilize knowledge of pain control & symptom management.
