Job summary

Continuum Hospice is seeking a self-driven Community Liaison to promote hospice services and build trust-based relationships with referral sources, physicians, and healthcare partners in the Oakland–Alameda County area. You’ll drive census growth by increasing awareness and preference for our specialized end-of-life care, while representing our mission with compassion and professionalism.

Schedule is Monday–Friday, 8 am–4:30 pm, and the role requires driving within the service area.

Qualifications

• Bachelor’s degree preferred; experience may substitute.
• Minimum two years of healthcare sales/marketing experience.
• Proven ability to build/maintain relationships with physicians and partners.
• Must be able to drive with reliable transportation.

Responsibilities

• Establish and nurture relationships with physicians and healthcare partners.
• Advocate for hospice services through outreach and education.
• Grow census by increasing community awareness and referrals.
• Drive referral growth via regular visits to physicians, discharge planners, SNFs, social workers.
• Lead initial conversations about hospice care with empathy and clarity.
