Job summary

UNITAS is recruiting a Clinical Pharmacist Specialist for a hospital setting. You will lead patient-centered medication therapy management, collaborate with physicians, nurses, and other clinicians, and ensure safe, effective, and timely medication services across clinics.

You will design stewardship programs, oversee Pharmacy and Therapeutics processes, and ensure compliance with URAC, HIPAA, and regulatory standards while supporting education and training for health care providers.

Qualifications

• Graduate of an accredited College of Pharmacy.
• Doctor of Pharmacy (PharmD) or other advanced degree (highly preferred).
• Completion of an accredited Pharmacy Residency (highly preferred).
• Clinical pharmacy experience (1+ years).
• Knowledge of contemporary clinical practice and service.

Responsibilities

• Lead clinical services to ensure safe, effective medication therapy in a hospital setting.
• Deliver MTM services and collaborate with multidisciplinary team.
• Monitor and review patient charts to detect drug-related problems and optimize therapy.
• Develop and oversee stewardship and surveillance programs (e.g., antimicrobial stewardship).
• Ensure compliance with URAC, HIPAA, and regulatory requirements.
